MyParentHQ is an iPhone app that helps adult children keep a parent's medications, allergies, conditions, appointments, and emergency contacts in one place, shared with siblings and caregivers by role. If you work with families at a care transition, you're welcome to share our free one-page handout. You don't need an account, and families decide who can see their information.

Who it's for

Families coordinating care for an aging parent: often adult children, sometimes across several states, who are suddenly being asked for medication lists, doctors' names, and paperwork they don't have in one place. The parent doesn't need an account or a smartphone.

What families use it for

What it isn't

For families who prefer paper

The MyParentHQ Companion Workbook uses the same categories as the app, and our free printable emergency information sheet is a single page families can fill in before they leave the building.

Helpful public resources to pair with it

For local services after discharge, families can use the Eldercare Locator, a free federal service (1-800-677-1116, weekdays 8 a.m. to 9 p.m. Eastern). For Medicare questions, every state's free SHIP counseling program is listed at shiphelp.org.
